• This offer is available to new patients only and cannot be combined with other offers, discounts, or insurance benefits
• If periodontal (gum) disease or other conditions are present, the basic cleaning may not be appropriate. In such cases, your dentist will discuss additional treatment options and associated costs before any work is performed
• Offers are subject to availability and may be modified or discontinued at any time without prior notice
• One offer per patient per household

4. No Doctor-Patient Relationship

Use of this website does not establish a doctor-patient relationship. A doctor-patient relationship is established only when you are seen in person at our office and treatment is initiated.
